ASQ INFORMATION
Ages and Stages Questionnaire or ASQ-3 is a set of questionnaires from 1 month to 5 ½ years old to follow children’s development. It has been used for more than 20 years to make sure children are developing well. It is called a screener because it looks at how children are doing in important areas, such as speech, physical ability, social skills, and problem-solving skills. ASQ-3 can help identify children’s strengths as well as any areas where they may need support.
They will answer each question “yes,” “sometimes,” or “not yet,” based on what the child is able to do now.

To answer each question, parents can try fun and simple activities with their child. These activities encourage the child to play, move around, and practice day-to-day skills.

The State of Alaska Early Intervention/Infant Learning Program is providing this resource at no cost to providers so that all children between birth and 5 years of age are periodically screened and possible delays identified early. PIC will provide physicians and other providers with ASQ-3 and ASQ-SE (screening for Social and Emotional Development) screening kits so that you can use this standardize tool to complete developmental screenings. The program includes an online database to score and save results over time. In addition, the ASQ Online has Family Access which allows programs to provide a link to patients to go on line and complete the screening which is selected according to the child’s age. The Family Access link can be on your programs website or emailed to your families as part of your established well baby checks. Parents complete the questionnaires online but the results are shared with the parents during their check up appointment. Referrals to PIC can be made directly from the ASQ Online program.
